You must complete all fields on the form. If completed correctly, you will receive a message saying it has been submitted. In addition, you will receive an email with your form responses. If you do not receive the submission message or email (check your spam), the information was not submitted. If you have any questions, please contact us.

A copy of the official registration and financial information may be obtained from the Division of Consumer Services by
calling (800-435- 7352) within the state of Florida. Registration does not imply endorsement, approval, or recommendation by the state.
